Private Property Damage Assessment Forms
Property owners affected by the recent flooding must complete a private property damage assessment form in order to be considered for potential financial assistance under the Ontario Disaster Relief Assessment Program (ODRAP).
Private property damage assessment forms should be completed and returned to the Municipality of Tweed as soon as possible.
Click here for the Private Property Damage Assessment form which needs to be completed in its entirety. Return forms to the municipal office at 255 Metcalf Street ASAP. Forms can be dropped off, faxed to 613-478-6457, or scanned and emailed to info@twp.tweed.on.ca.
